Please call our office in order to become "web-enabled" to start using the portal. Once you become web-enabled, you will receive an email within 3 minutes with a username and password. You are not required to fill out your past medical history, social and family history unless you would like to update your file.

Please call our office and schedule an appointment, and request access to the Patient Portal Page prior to your first appointment. Please fill in all the fields on the portal as well.
